ACP is the process of an individual deciding on and ideally documenting their wishes and preferences for a time in the future when they may lose the capacity to decide or the ability to communicate their wishes, such as during periods of illness or in developing dementia. Discussion topics: 1. Introduction to ACP and definition of terms 2. Explanation of procedures, including:
forms of delivery - face to face, video conference or phone different settings - one on one or family conference, patient or social worker, and interpreter involvement - how to form an effective team.
3. Ethical dilemmas:
Boundaries for the interpreters Dealing with family members interrupting or providing contradictory responses Handling family members giving instructions during cognitive testing.
